The fatal incident took place during a support weapon and hand grenade training session at the Basic Grenade Training Site. The training was part of the defence assistant enhancement course class 1 Series 78/23.
Initial Casualties and Treatment

One RMAF personnel, identified as a student on the course, died at the scene. Two trainers sustained injuries during the exercise. They received initial treatment at Gemas Army Hospital before being referred to Segamat Hospital for further care.
Second Fatality and Ongoing Investigation

Tragically, one of the injured trainers later died while undergoing treatment at Segamat Hospital. The RMAF has notified the next of kin of all involved personnel and has handed the case over to the police. An RMAF Investigation Board will be established to determine the cause of the incident.
