Discovery of Tampered Report

Madinah's revelation came to light when Datuk Nor Salwani Muhammad, the coordinator of the 1MDB audit team, presented her with two versions of the audit report in March 2017. These were identified as the "original copy" and the "amended copy." This encounter, which took place just before Madinah was to be briefed by the National Audit Department's 1MDB audit team on the report submitted to Parliament’s Public Accounts Committee (PAC), left her shocked.
Orders to Amend and Destroy

The shock stemmed from the information that her predecessor, Tan Sri Ambrin Buang, had received instructions in 2016 to alter the 1MDB audit report before its submission to the PAC. Furthermore, an official within the audit team was directed to "destroy" all existing copies of the original report. Nor Salwani Muhammad had, however, managed to retain one copy of the original report despite the directive to eliminate all such versions.
Lack of Immediate Action

Upon learning of the existence of two versions and the instructions given, Madinah stated she was surprised but did not immediately inquire about who had issued the orders to Ambrin. She explained that at the time of the initial conversation with Nor Salwani in March 2017, officers were waiting for a briefing, limiting the time available for discussion. She later received a comprehensive briefing on the report's amendment in 2018.
